2012 Discipline Guidelines ( pdf ) Each student is expected to follow the school policy, bus rules, and guidelines set by the Board of Education, Administration, and State of Ohio. Contract for Parent to Provide Transportation ( online / pdf ) The purpose of this form is to provide a contract between the parent/guardian and the Board of Education for the parent/guardian to provide transportation. Change of Information for Transportation ( online / pdf ) This form is to be used to request a temporary or permanent change to transportation needs of students. Please use a separate form for each student. Requests must be submitted 48 hours prior (not including weekends and non-school days). You will be notified if your request has NOT been approved. NO phone calls will be accepted for changes. Form must be submitted. Changes in student transportation will NOT be approved for play dates or social functions. Registration for Non-Public Transportation ( online / pdf ) This form is required in order for your son/daughter to receive transporation from the Sugarcreek Local School District to a non-public school. Emergency Medical ( pdf only ) Each student riding Sugarcreek transportation must have an Emergency Medical Form on file with the transportation office. Please print this form, complete it by hand (including signature), and send it to the Sugarcreek Transportation office. |
